Case Manager - follow-up and outcomes (Minneapolis, MN)

Rivermark Harbor Care Services provides in-home and community support in Minneapolis, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Minneapolis. • Applications are reviewed on a rolling basis.
