Case Manager - follow-up and outcomes (Minneapolis, MN)

We are growing our team at Clearpath Ivory Health Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Minneapolis. • We will tell you what to expect at each step of the hiring process.
