Case Manager - follow-up and outcomes (Minneapolis, MN)

We are growing our team at Amberfield Wren Care Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. We train and promote from within. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Minneapolis, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
