Case Manager - follow-up and outcomes (Minneapolis, MN)

Rivermark Glen Health Group is a healthcare provider in Minneapolis with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with electronic records. • Bilingual English and Spanish.
