Case Manager - follow-up and outcomes (Minneapolis, MN)

Clearpath Cobalt Property Management is looking for a Case Manager. The job centres on follow-up and outcomes for vendor invoices, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Minneapolis. What you will do • Help with applications and appointments for vendor invoices. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Minneapolis. • Applications are reviewed on a rolling basis.
