Case Manager - follow-up and outcomes (Minneapolis, MN)

Clearpath Ashton Health is a healthcare provider in Minneapolis with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Minneapolis. • We will tell you what to expect at each step of the hiring process.
