Case Manager - follow-up and outcomes (Milwaukee, WI)

Harborlight Fox Learning Group is an education provider in Milwaukee serving students and families, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for school calendars. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Milwaukee, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
