Case Manager - follow-up and outcomes (Milwaukee, WI)

We are growing our team at Highbridge Works and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Milwaukee. What you will do • Connect clients with partner agencies. • Help with applications and appointments for resident events. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Milwaukee, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
