Case Manager - follow-up and outcomes (Milwaukee, WI)

We are growing our team at Foxglove Drift Civic Services Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 55 people based in Milwaukee. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for constituent requests. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with constituent requests. • Bilingual English and Spanish. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Milwaukee. • We reply to every applicant.
