Case Manager - follow-up and outcomes (Milwaukee, WI)

Westbrook Vale Housing is looking for a Case Manager. The job centres on follow-up and outcomes for maintenance stores, and it would suit someone who likes steady, practical work. What you will do • Help with applications and appointments for maintenance stores. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Milwaukee, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
