Case Manager - follow-up and outcomes (Milwaukee, WI)

Stonegate Sparrow Civic Services supports public agencies in Milwaukee with budgeting, records and programmes, and we need a Case Manager to lead our follow-up and outcomes. We follow procedures carefully and explain them clearly. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for procurement. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with procurement. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Milwaukee, with some flexibility after the first few months. • We reply to every applicant.
