Case Manager - follow-up and outcomes (Milwaukee, WI)

Bluepeak Granite Community Partners is a Milwaukee charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Milwaukee. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for client intake. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Milwaukee. • Applications are reviewed on a rolling basis.
