Case Manager - follow-up and outcomes (Milwaukee, WI)

Fernwood Linden Health is looking for a Case Manager. The job centres on follow-up and outcomes for electronic records, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Milwaukee, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
