Case Manager - follow-up and outcomes (Miami, FL)

Bluepeak Cedar Health is a healthcare provider in Miami with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for clinic supplies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Miami. • Applications are reviewed on a rolling basis.
