Case Manager - follow-up and outcomes (Miami, FL)

Bright Union Health Partners runs clinics and patient services around Miami. We are hiring a Case Manager to take on follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Miami. • We will tell you what to expect at each step of the hiring process.
