Case Manager - follow-up and outcomes (Miami, FL)

Foxglove Ivory Health is looking for a Case Manager. The job centres on follow-up and outcomes for referrals, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Miami. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Miami, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
