Case Manager - follow-up and outcomes (Miami, FL)

Fernwood Maple Community Partners Group is looking for a Case Manager. The job centres on follow-up and outcomes for client intake, and it would suit someone who likes steady, practical work. Flexibility and kindness are expected. What you will do • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Miami, with some flexibility after the first few months. • We reply to every applicant.
