Case Manager - follow-up and outcomes (Miami, FL)

Redstone Forge Care Services supports families and individuals in Miami with personal care and coordination, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for caregiver training. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Miami, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
