Case Manager - follow-up and outcomes (Memphis, TN)

We are growing our team at Clearpath Iron Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 140 people based in Memphis. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Memphis, with some flexibility after the first few months. • We reply to every applicant.
