Case Manager - follow-up and outcomes (Madison, WI)

Northwind Glen Health provides primary and specialty care in Madison. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for insurance authorisations. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Madison. • Applications are reviewed on a rolling basis.
