Case Manager - follow-up and outcomes (Madison, WI)

We are growing our team at Lumen Wilder Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for enrolment. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Madison. • We reply to every applicant.
