Case Manager - follow-up and outcomes (Louisville, KY)

Silverline River Housing provides affordable homes and support services around Louisville, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 525 people based in Louisville. What you will do • Help with applications and appointments for lease compliance. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Louisville, with some flexibility after the first few months. • We reply to every applicant.
