Case Manager - follow-up and outcomes (Louisville, KY)

Ironwood Vale Community Partners is a Louisville charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, Monday to Friday. • On site in Louisville. • We will tell you what to expect at each step of the hiring process.
