Case Manager - follow-up and outcomes (Little Rock, AR)

Harborlight Hollow Care Services Group is looking for a Case Manager. The job centres on follow-up and outcomes for caregiver training, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish.
