Case Manager - follow-up and outcomes (Little Rock, AR)

We are growing our team at Stonegate Pike Property Management Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for vendor invoices. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with vendor invoices. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Little Rock, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
