Case Manager - follow-up and outcomes (Las Vegas, NV)

Foxglove Junction Community Partners Group is a nonprofit in Las Vegas supporting families with food, housing and job help. We are hiring a Case Manager to take on follow-up and outcomes. Flexibility and kindness are expected. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, Monday to Friday. • On site in Las Vegas. • We reply to every applicant.
