Case Manager - follow-up and outcomes (Las Vegas, NV)

Stonegate Ivory Health Group is looking for a Case Manager. The job centres on follow-up and outcomes for patient education, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Las Vegas. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Las Vegas. • We will tell you what to expect at each step of the hiring process.
