Case Manager - follow-up and outcomes (Knoxville, TN)

Redstone Forge Care Services Partners provides in-home and community support in Knoxville, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Full-time, with a steady weekday schedule. • On site in Knoxville, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
