Case Manager - follow-up and outcomes (Knoxville, TN)

Harborlight Ivory Health Associates is a healthcare provider in Knoxville with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Knoxville. • Applications are reviewed on a rolling basis.
