Case Manager - follow-up and outcomes (Kansas City, MO)

We are growing our team at Clearpath Wren Health and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 942 people based in Kansas City. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations.
