Case Manager - follow-up and outcomes (Jacksonville, FL)

We are growing our team at Fernwood Basin Civic Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Jacksonville. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for budget submissions. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with budget submissions. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Jacksonville. • We will tell you what to expect at each step of the hiring process.
