Case Manager - follow-up and outcomes (Jacksonville, FL)

Silverline Sparrow Care Services is a social-care provider serving older adults and people with disabilities around Jacksonville. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Jacksonville, with some flexibility after the first few months. • We reply to every applicant.
