Case Manager - follow-up and outcomes (Jackson, MS)

We are growing our team at Fernwood Summit Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for learning materials. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Jackson. • We reply to every applicant.
