Case Manager - follow-up and outcomes (Jackson, MS)

Lumen River Learning Group is an education provider in Jackson serving students and families, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 49 people based in Jackson. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for attendance records. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Jackson. • We will tell you what to expect at each step of the hiring process.
