Case Manager - follow-up and outcomes (Indianapolis, IN)

Greenridge Haven Learning Group is an education provider in Indianapolis serving students and families. We are hiring a Case Manager to take on follow-up and outcomes. Collaboration between teachers and staff is strong. What you will do • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Indianapolis. • Applications are reviewed on a rolling basis.
