Case Manager - follow-up and outcomes (Indianapolis, IN)

Greenridge Glen Health is a healthcare provider in Indianapolis with clinics and support teams, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Indianapolis, with some flexibility after the first few months. • We reply to every applicant.
