Case Manager - follow-up and outcomes (Indianapolis, IN)

Ironwood Glen Civic Services supports public agencies in Indianapolis with budgeting, records and programmes, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 37 people based in Indianapolis. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for public records. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Indianapolis. • We reply to every applicant.
