Case Manager - follow-up and outcomes (Houston, TX)

Stonegate Loom Learning Group is looking for a Case Manager. The job centres on follow-up and outcomes for learning materials, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for learning materials. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Houston, with some flexibility after the first few months. • We reply to every applicant.
