Case Manager - follow-up and outcomes (Houston, TX)

Silverline Haven Community Partners runs community programmes across Houston. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 47 people based in Houston. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Houston. • Applications are reviewed on a rolling basis.
