Case Manager - follow-up and outcomes (Houston, TX)

Summit Pine Housing manages affordable housing communities across Houston, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for waiting lists. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Houston. • We reply to every applicant.
