Case Manager - follow-up and outcomes (Houston, TX)

Lumen Vale Community Partners runs community programmes across Houston. We are hiring a Case Manager to take on follow-up and outcomes. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Houston. • Applications are reviewed on a rolling basis.
