Case Manager - follow-up and outcomes (Houston, TX)

We are growing our team at Silverline Pine Community Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 142 people based in Houston. What you will do • Help with applications and appointments for food distribution. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Houston, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
