Case Manager - follow-up and outcomes (Houston, TX)

Stonegate Wilder Property Management manages apartment communities around Houston. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for unit turnovers. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers. Working arrangement • Full-time, on a regular day shift. • On site in Houston, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
