Case Manager - follow-up and outcomes (Honolulu, HI)

Bluepeak Amber Property Management looks after rental homes across the Honolulu area, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 905 people based in Honolulu. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for resident events. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Honolulu. • Applications are reviewed on a rolling basis.
