Case Manager - follow-up and outcomes (Honolulu, HI)

Westbrook Glen Health Group is a healthcare provider in Honolulu with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Honolulu. • Applications are reviewed on a rolling basis.
