Case Manager - follow-up and outcomes (Honolulu, HI)

Ironwood Cobalt Property Management looks after rental homes across the Honolulu area, and we need a Case Manager to lead our follow-up and outcomes. Maintenance and leasing staff share information every day. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for unit turnovers. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Honolulu. • We reply to every applicant.
