Case Manager - follow-up and outcomes (Hartford, CT)

Northwind Juniper Health Group provides primary and specialty care in Hartford. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Hartford, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
