Case Manager - follow-up and outcomes (Hartford, CT)

We are growing our team at Lumen Haven Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. Low staff turnover is a goal we take seriously. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for meal programmes. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Hartford. • We reply to every applicant.
