Case Manager - follow-up and outcomes (Hartford, CT)

We are growing our team at Amberfield Harbor Care Services Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 15 people based in Hartford. What you will do • Help with applications and appointments for client records. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Hartford. • We reply to every applicant.
