Case Manager - follow-up and outcomes (Hartford, CT)

Summit Nest Community Partners is a Hartford charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Hartford. • Applications are reviewed on a rolling basis.
